What is Proventil?

Proventil is a beta-2 adrenomimetic agonist. This drug belongs to the group of expectorant pharmaceuticals, which are used to treat diseases of the respiratory system. The active ingredient, salbutamol (albuterol), has a significant bronchodilator effect, concentrating in the blood vessels of the bronchi. It has an invaluable property to prevent and interrupt the emerging bronchospasm. The medicine is able to reduce the resistance that occurs in the respiratory tract, while the vital capacity of the lungs increases. The drug facilitates the work of the lower parts of the respiratory system – the lungs and bronchi.

Indications for use

Bronchial asthma:

  • relief of asthma symptoms when they occur;
  • prevention of attacks of bronchospasm associated with exposure to an allergen or caused by exercise;
  • use as one of the components for long-term maintenance therapy of bronchial asthma.

Proventil can also be used in other chronic lung diseases associated with reversible airway obstruction, including chronic obstructive pulmonary disease (COPD), chronic bronchitis, pulmonary emphysema.

Bronchodilators should not be the sole or main component of the therapy of unstable or severe asthma. In the absence of a reaction to salbutamol in patients with severe asthma, it is recommended to administer glucocorticosteroid therapy in order to achieve and maintain control of the disease. A lack of response to salbutamol therapy may indicate the need for urgent medical advice or treatment.

Instructions for use

The drug is used for inhalation only.

To inhale medication, you need to:

  • shake the spray can;
  • exhale the air from the lungs;
  • insert an aerosol mouthpiece into your mouth, clasping it with lips on all sides;
  • pressing the bottom of the can, take a deep breath;
  • hold your breath for as long as possible.

Usually, a patient requires 1 inhalation. The effect is observed in about 5 minutes.

If 1 dose of the drug is not enough, then repeat the procedure. At the same time, the interval between two doses should be at least 30 seconds.

To prevent an attack during physical activity or contact with an allergen, inhale 2 doses of the drug.

With prolonged maintenance therapy, inhale 2 doses 4 times a day.

Adults and children older than 4 years of age: 2 inhalations every 4–6 hours as needed; 1 inhalation every 4hrs may suffice.

Exercise-induced bronchospasm: 2 inhalations 15–30 minutes before exercise.

Side effects

  • Nervous system: anxiety, tremor (usually of the hands), tension, dizziness, irritability, sleep disturbances, headache, short-term seizures;
  • Digestive system: nausea, irritation or dryness in the mouth or throat, vomiting, loss of appetite;
  • Circulatory system: tachycardia (including in the fetus during pregnancy), heartbeat, arrhythmia, increased or decreased blood pressure, dilated peripheral vessels, heart failure, myocardial ischemia, cardiopathy;
  • Other: pharyngitis, bronchospasm (caused by hypersensitivity to salbutamol), sweating, difficult urination, blood glucose increase level of free fatty acids, dose-related hypokalemia, development of psychic and physical drug dependence, allergic reactions such as facial edema, erythema, wheezing.

Absolute contraindications:

  • age up to 1.5 years;
  • hypersensitivity to any component of the drug.

With caution: pregnancy and lactation, a history of tachyarrhythmias, severe chronic heart failure, hypertension, thyrotoxicosis, pheochromocytoma.

  • Salbutamol increases the cardiotropic state of thyroid hormones, the activity of stimulants of the central nervous system;
  • Ephedrine and theophylline increase the toxic effects of the bronchodilator;
  • Prostaglandin synthesis inhibitors, corticosteroids, MAO blockers and tricyclic antidepressants increase the likelihood of complications from the circulatory system;
  • Use with inhalation anesthesia can cause severe ventricular arrhythmias;
  • Salbutamol reduces the effectiveness of antihypertensive drugs, beta-blockers (including ophthalmic forms), antianginal effect of nitrates;
  • Salbutamol increases the risk of glycoside intoxication when used together.

Restrictions on the use of Proventil:

Tachyarrhythmia, ischemic heart disease, severe arterial hypertension, heart disease, myocarditis, severe heart failure, aortic stenosis, thyrotoxicosis, diabetes mellitus, pheochromocytoma, severe renal or/and hepatic failure.

Use during pregnancy and lactation:

The use of the inhaler is contraindicated during pregnancy and breastfeeding.

An overdose of salbutamol can cause a decrease in blood pressure, tachycardia (pulse rate up to 200 beats per minute), ventricular flutter, increased cardiac output, acidosis, hypoxemia, hyperglycemia, hypokalemia, headache, muscle tremor, agitation, convulsions, hallucinations develop.

Treatment: stop using the medication and conduct symptomatic treatment. The use of selective beta-adrenergic blockers in patients with bronchial asthma due to the risk of developing a severe bronchospastic reaction requires extreme caution.
